Membership Form

We apply for the District Level Membership of Uttar Pradesh Cerebral Palsy Sports and Welfare Trust, a State Level body registered with Government of Uttar Pradesh which is affiliated to CPSFI, a National Sports Federation (NSF) recognised by Ministry of Youth Affairs and Sports, Government of India.

We endorse and accept the principles and policies in CPSFI's Constitution (check www.cpsfi.org). We comply with the terms of the CPSFI Constitution, including the payment of membership fees.

We enclose a copy of our Constitution, Registration Certificate and a report on the situaton in our District regarding promoting sports for people with Cerebral Palsy.
